Requirements:
- For group memberships you must submit the names of the persons under your group membership. Name, email address, phone numbers, and an address must be provided to the BNA.
- If there is a change with a person in your company that is no longer apart of your group, you may add another person to the membership to replace the old exiting member. (Only if your company paid for the whole membership).
